Eye protection fails by mismatch more than by absence: glasses worn where dust needed a seal, a shield worn alone where the eye needed its own barrier. The three tiers exist because hazards reach the eye by different routes — sort the route and the tier picks itself.

The tiers
Safety glasses (spectacle-style, tested under EN 166, typically to impact grade F — 45 m/s particles) stop things travelling at the eye in a straight line: chips, swarf, wire ends. They're light, fog least, and are rightly the site default. What they can't do is seal — dust, mists and splashes go round them.
Goggles add the seal. Ventilated goggles handle coarse dust; sealed or indirect-vent versions handle fine dust, liquid splash and the grinding tasks that shower from odd angles — typically at impact grade B (120 m/s). The cost is fogging and comfort, which is why goggles get taken off exactly when they shouldn't be; anti-fog coatings and proper venting are what make a goggle wearable enough to survive the shift.
Face shields protect the face — and the standard's quiet rule is that a shield is usually secondary protection: worn over glasses or goggles, not instead. Grinding, wire brushing, chemical decanting and forestry visors all follow the same logic: the shield takes the spray and the fragments; the sealed layer beneath guards the eye itself.

Questions
When the hazard travels around lenses rather than straight at them: fine dust, powders, liquid splash and mist, or grinding debris from odd angles. Glasses stop direct particles; only a sealed goggle stops what floats and splashes. The seal is the protection, so fit and anti-fog matter.
Usually not — shields are secondary protection for the face, worn over safety glasses or goggles. Fragments and splash can bounce behind a shield's open edges, so the eye needs its own sealed or impact-rated layer beneath for most tasks that justify a shield at all.
EN 166 impact grades: F withstands 45 m/s particles (typical for spectacles), B withstands 120 m/s (typical for goggles and shields), A 190 m/s (shields for the harshest tasks). A 'T' suffix (FT, BT) means the grade holds at temperature extremes.
